Incident Report Form
For the health and safety of our members and friends, UUCCH provides first aid supplies, including Narcan and Automated External Defibrillators (AED) to assist with perceived health emergencies. We rely on staff and adult volunteers to responsibly call 9-1-1 when needed. Any significant medical incident or injury on the UUCCH property requiring first aid or medical treatment should be reported on the Incident Report Form within 24 hours or as soon as possible thereafter, entered into the records of the church and communicated to the agency providing insurance for the church. The Form should be filled out by the injured party or primary witness.
Forms are available in the first aid kits and on our website here. The completed form can be left for the church administrator in the mailbox in Unitarian House or sent electronically to info@uucch.org.
