Grant Application Request Form
Per the UUCCH Board policy on grants, staff and volunteers who wish to pursue an external grant must complete this request form.
In order to be considered at the next monthly Board meeting, the form must be submitted at least two weeks before the meeting date. Applicants are always welcome to consult with the Minister and/or Executive Committee in advance of submitting their proposal.
The Board will review requests in consultation with the relevant Board Advisory Committees and will notify the applicant whether the proposal is approved or denied.
